For many of us, the gynecologist is where our breast health journey quietly begins. It’s the office where mammograms are ordered, breast exams are done, and family history questions are (hopefully) asked.
Yet so many women, including me, walk in and out of those appointments without understanding breast density, genetic risk, or even how to pay attention to our own breasts.
This page was born from a long, honest interview with Dr. Staci Tanouye, OB/GYN and Learn Look Locate Medical Advisor. Together, we talked about everything I wish I had understood before hearing the words “you have breast cancer”: dense breasts, callbacks, rare breast cancers, genetic testing, oophorectomy, and what it really means for a gynecologist to listen.

Most women see their gynecologist long before they ever meet a breast radiologist, oncologist, or surgeon. That means so much of what happens — or doesn’t happen — in those visits shapes whether a cancer is caught early or found later when symptoms are harder to ignore.
“I walked into my gynecology appointments assuming that if something important was there, someone would tell me. What I didn’t realize was how much depended on time, guidelines, insurance, and my own questions. I didn’t even know what to ask.”

Your annual exam is not just a pap smear. It is a critical moment to say:
“My breast feels different here.”
“This pain hasn’t gone away.”
“My mammogram letter said I have ‘dense breasts.’ What does that mean?”
“My mom had cancer at 48. Does that change anything?”
You deserve more than reassurance. You deserve a conversation.
Breast Density:
A Hidden Risk Factor That Changes Everything
“My own diagnosis came with a shock: I learned I had extremely dense breasts (Category D) — and that this might have hidden my walnut-sized tumor on years
of 2D mammograms.”

No one had ever sat me down and said:
“You have extremely dense breasts.”
“This is a risk factor.”
“2D mammograms alone may not be enough.”
Instead, I got letters that said “dense breasts, normal mammogram” — and I went on with my life.

Dr. Staci helped unpack why this happens.
Breast density has only recently been standardized in radiology reports. It must now be labeled as A, B, C, or D.
For years, guidelines for what to do about breast density were vague — and only became clearer in 2025.
OB/GYNs are still catching up on how to counsel patients about what dense breast tissue really means.
Insurance often hasn’t caught up either.
What are the breast density categories — and what do they mean?
A — Almost entirely fatty
B — Scattered fibroglandular tissue
C — Heterogeneously dense
D — Extremely dense
MRI, Fast MRI, and the Hard Truth About Insurance
Once you know you have dense breasts, the next question is:
What now?
For years, the guidelines around dense breasts were vague.
“Consider supplemental screening” left both doctors and patients stuck.
In 2025, this became clearer — especially for those with Category D (extremely dense) breasts.
For Category D, guidelines now recommend additional screening, preferably MRI.
For Category C, the language still says “consider” — which sounds gentle, but carries major implications.
“Consider” doesn’t help much when you’re trying to navigate real life.
Women immediately start asking:
- Will insurance cover an MRI?
- Will the bill be $0, $400, or $2,000?
- How often should I do this — every year or every few years?
Dr. Staci describes the counseling challenge clearly:
“When guidelines say ‘consider,’ it’s wishy-washy — especially for insurance. I can talk to patients about recommendations, but whether they can actually follow them financially and logistically is a whole different story.”

Even when MRI is covered, deductibles and plan structures can leave women with large, unexpected bills. This is one of the most painful disconnects in breast health: women are told to be proactive — then financially punished for trying.
Fast Breast MRI: A Glimpse of Hope
Fast Breast MRI is a shorter, lower-cost version of a traditional breast MRI that is becoming more available in some centers. While availability and insurance coverage still vary, it represents an important step toward making advanced screening more accessible for women with dense breasts.
Breast Density Confusion:
What C and D Really Mean for You
One of my biggest questions to Dr. Staci was simple:
Do people even understand what Category C and D mean?
Her answer was honest:
“It’s not that patients can’t understand the categories. It’s that they don’t know what they mean for them. Do I really need more imaging? How often? Is it worth the money? Those are hard decisions.”
Here’s what we know:
- Over 40, about 50% of women will be C or D.
- Roughly 40% are C (heterogeneously dense).
- About 10% are D (extremely dense).
- Category D now comes with a more firm MRI recommendation.
- Category C lives in a gray zone: “consider additional imaging.”
This matters for you because:
- You may be walking around in Category C and never know supplemental imaging is even a conversation.
- Your OB/GYN may be trying to explain risk, but stuck in a 15-minute slot. When guidelines are soft (“consider”), insurance tends to say “no”.
Questions for your next appointment:
- “Can you tell me my breast density category (A, B, C, or D)?”
- “Based on my density and risk factors, what extra screening would you recommend if cost were no issue?”
- “What are my realistic imaging options if my insurance does not cover MRI?”
Clarity Matters
Callbacks, Biopsies, and the
Emotional Roller Coaster
The emotional side of screening doesn’t start with a diagnosis. It starts with a callback.
Dr. Staci reminded us that:
For first mammograms, especially in younger women, the callback rate is high.
Most callbacks do not lead to biopsy.
Most biopsies do not lead to a cancer diagnosis.
But none of that logic cancels the anxiety when you get that phone call.
“For so many women, the moment they are told ‘we need more imaging’ is the moment their world starts to shake. Even if it ends up benign, the fear stays in the body.” — Cynthia, Stage 2B
Biopsies themselves are usually handled by breast radiologists, not OB/GYNs. In many practices:
The radiologist both performs the biopsy and communicates results.
OB/GYNs then step in for referrals and next steps.
This difference in workflow can confuse patients — especially when they don’t know who will call with what result, or when.
You deserve:
- To know why you’re being called back
- To understand what type of biopsy is being done
- To have someone acknowledge how emotionally difficult this step is.
- Knowledge doesn’t erase fear, but it gives you something to hold onto while you wait.
Rare Breast Cancers and the
Danger of Dismissal
Paget’s disease of the breast, inflammatory breast cancer (IBC), and even some cases of lobular breast cancer can present very differently from the classic “lump” many of us are taught to look for.
They can start with:
- Persistent nipple itching or flaking
- Skin changes that look like eczema or a rash
- Sudden warmth or swelling in one breast
- A breast that feels heavier or different almost overnight
- Skin that doesn’t improve with typical treatments
I asked Dr. Staci directly: are gynecologists being adequately trained to recognize these?
“There’s an understanding these rare forms exist, but they’re rare. In 17–18 years of training and practice, I can count on one hand how many I’ve personally seen. That makes them harder to recognize in real time.”
She used the term “zebra diagnosis” — rare conditions that are like needles in a haystack. As a clinician and as a patient who herself had a rare diagnosis (outside of the breast), she’s seen both sides:
Sometimes the system truly can’t “find” a rare disease despite effort.
That is different from a doctor dismissing symptoms and refusing to investigate.
“The real problem is when clinicians are dismissive,” she said. “If I don’t know what I’m seeing, my job is to raise my hand and ask for help — refer to a specialist, get more imaging, bring in someone else. Not to tell the patient it’s nothing and send them away.”
What you can do if something feels wrong:
- Keep a photo log of skin changes.
- Write down symptoms and how long they’ve lasted.
- Ask directly: “Could this be Paget’s disease or inflammatory breast cancer?”
- If you feel brushed off, seek a second (or third) opinion.
You are not overreacting. You are advocating.
Genetic Risk
Screening by 25: What
Should Be Happening
(But Often Isn’t)
I asked a personal question in the interview:
“I have a 25-year-old daughter. I had cancer. My father had lung cancer. My grandfather had colon cancer. Should she be getting genetic testing? How does that even work?”
Dr. Staci shared that current guidelines recommend genetic risk screening by age 25. That doesn’t mean everyone gets a blood test or saliva test at 25 — it means:
At 25, someone should sit down and review family history and personal risk factors.
Certain “red flags” (breast cancer under 50 in a first-degree relative, multiple relatives with breast /ovarian/pancreatic cancer, etc.) trigger a recommendation for genetic testing.
Some electronic medical records now include built-in algorithms that flag risk based on patient intake forms.
But she also acknowledged the reality:
“Not every practice is doing this consistently. Not every EMR is using those tools. And not every patient is being asked for a detailed family history early on.”
This gap means:
Many women with clear red flags are not being referred.
Genetic testing is often introduced after a cancer diagnosis rather than before.
Families lose precious time that could have been used for prevention.
“I was never given a genetic risk questionnaire until after my diagnosis. To think this could have been started at 25 is still hard for me to process.”

Insurance, Flat Fees, and the Reality of Genetic Testing
Even when a woman is identified as a good candidate for testing, the next barrier is familiar: insurance.
Dr. Staci walked through the complexity:
- Different insurance companies follow different risk models.
- One family history might “qualify” for coverage in one plan but not another.
- When patients call their insurers, they often get vague or incorrect answers.
- To try to offset this uncertainty, many genetic testing companies now:
- Use family history forms (originally paper, now electronic) to determine risk.
- Cap out-of-pocket costs at a flat fee — often around $250–$300 — if insurance denies coverage.
This helps, but doesn’t completely solve:
- Patients still don’t know their final bill beforehand.
- Not everyone can afford even the capped amount.
- The burden of chasing coverage still falls on the patient.
“Our healthcare system is broken,” Dr. Staci said frankly. “I wish I could just say, ‘You have this history; your insurance will cover it.’ But I can’t.”
What you can do:
Ask your OB/GYN if they partner with a specific genetic testing company.
Ask if that company has a maximum out-of-pocket policy.
Request a referral to a genetic counselor to review options in detail.
Genetics won’t answer everything. But for many women and families, it can be life-altering information.
Breast Self-Awareness
and the Push-and-Drag
Method
For years, the message around self-exams changed.
One month we were told, “Do a self-exam every month.”
Then, “That doesn’t lower mortality; don’t over-focus on exams.”
Now, the language has softened into breast self-awareness.
That shift left a lot of us confused.
“I never did self-breast checks. I wasn’t taught how, and I didn’t think it mattered since I had no family history. I now understand self-awareness isn’t a test — it’s a relationship with your own body.”

How Dr. Staci Teaches Breast Awareness
In the clinic, she starts clinical breast exams in the late teens or early 20s.
Uses the visit to demonstrate how she examines the breast. Prefers a method called push-and-drag over circular patterns, especially in dense breasts.
The Push-and-Drag Method
Think of the breast as a circle. Start in the armpit and work in sections. Push down and drag your fingers toward the nipple. Move around the full circumference, section by section. Do this lying down and standing up (breast tissue moves differently in each position).
What you’re feeling for:
Normal lumpy breast tissue will move and blend under your fingers. Abnormal areas will feel like a little rock or pebble you have to go over — they don’t blend or move with the rest.
For the area behind the nipple, Dr. Staci herself often adds:
A gentle pinching motion up and down, side to side, to feel tissue under the areola.
You don’t have to be perfect. You’re not trying to catch every microscopic change. You’re trying to learn what “typical” feels like, so your brain and fingers will notice when something shifts. Most lumps are benign.
But recognizing a change is how you start the right conversation sooner.
When Pain Is Not the Symptom:
Understanding How Breast
Cancer Feels
One of the most surprising truths for patients is this:
“Breast cancer is painless in the vast majority of people.”
Dr. Staci emphasized that when someone comes in with breast pain, most of the time it is not cancer.
That doesn’t mean pain should be ignored, but it does mean:
A new, hard, immobile lump with no pain is far more concerning than generalized breast tenderness.
Painless changes can be overlooked because they aren’t screaming for attention.
This is why pairing:
- Regular imaging
- Breast awareness
- Honest conversations about density is so important.
- Pain is only one small part of a much bigger picture.
“My tumor was painless. I was working out, living my life, and had no idea there was something growing inside me. That’s how little I understood.”

Young Women, Dense Breasts, and Late Diagnoses
We are seeing more and more young women — in their 20s and 30s — being diagnosed with breast cancer. Many are stage II or III at diagnosis, and some progress to metastatic disease within a few years.
Some themes Dr. Staci and I discussed:
- Young women are often told they’re too young for cancer.
- Dense breast tissue is extremely common in this group.
- Traditional guidelines didn’t always prioritize this age range.
- Many organizations shifted away from promoting structured monthly self-exams, which left some young women uncertain about what to do.
“Every day online, I meet a new young woman who found a lump and had to fight to be taken seriously. Many are BRCA-positive and didn’t know it until after they were diagnosed. It breaks my heart.”

For young women especially, the message from this page is:
Your age does not disqualify your concern.
If you feel a new, distinct, persistent change, you deserve imaging.
If you are dismissed, it is okay — and necessary — to seek another opinion.
Oophorectomy, Surgical Menopause, and Individualized Risk Reduction
At 52, when I was diagnosed with ER-positive breast cancer, I was not fully in menopause. After treatment, my gynecologist brought up something I had never seriously considered: oophorectomy — removing my ovaries to lower estrogen and reduce my recurrence risk.
“When I learned that removing my ovaries could reduce my recurrence risk by around 50%, it was a powerful moment. My gynecologist did the surgery. It was outpatient, and while it wasn’t nothing, I felt a deep peace afterward knowing I had taken that step.”

Dr. Staci walked through the data:
For BRCA1/2 carriers who have not had breast cancer, risk-reducing salpingo-oophorectomy has been shown to lower breast cancer risk, with some studies reporting reductions of up to 68%. However, when data are averaged across mutation type and timing of surgery, the more consistently supported risk reduction is closer to the 40–50% range.
For context, risk-reducing bilateral mastectomy lowers breast cancer risk by approximately 90–95%, highlighting that different preventive strategies offer very different levels of risk reduction and should be discussed individually.
- In non-carriers who have already had an ER-positive cancer, surgical removal of the ovaries offers risk reduction similar to medical ovarian suppression (e.g., GnRH agonists like Lupron).
Surgical menopause is permanent, unlike medical suppression.
- There are trade-offs:
- Removing ovaries can reduce cancer risk, but:
- Increases the risk of osteoporosis and cardiac issues if unmanaged.
- Can cause intense hot flashes, night sweats, and sleep issues.
- However, new non-hormonal medications for vasomotor symptoms are giving women more options.
At the Menopause Society meeting, Dr. Staci heard emerging research that in BRCA previvors (no cancer yet):
- Removing tubes, ovaries, and possibly the uterus may allow for estrogen-only hormone therapy, which may not increase breast cancer risk and might even be protective in some contexts.
- The key isn’t that one option is “right” — it’s that the decision is individualized, carefully weighed, and supported over time.
The Future of Screening:
Artificial Intelligence, MRI,
and Less Contrast
We also talked about artificial intelligence — not as a buzzword, but as a practical tool that’s already being studied.
Dr. Staci shared data she’d just heard:
Studies comparing MRI with contrast vs MRI without contrast plus Artificial Intelligence show encouraging overlap.
If Artificial Intelligence can reliably analyze non-contrast MRI, it may allow:
- Less exposure to gadolinium-based contrast.
- Lower cost.
- Better access for more women.
Artificial Intelligence may:
- Help radiologists detect subtle changes in dense tissue.
- Reduce the need for repeated contrast-based imaging.
- Make advanced screening more scalable globally.
We don’t have all the answers yet. But we do know the direction:
better detection, less harm, and more access.
Humility, Second Opinions, and the Care You Deserve
As we closed our conversation, one theme stood out:
HUMILITY
“Our role as OB/GYNs,” Dr. Staci said, “is not just to screen and detect. It’s to listen. If we don’t know what something is, our job is to ask for help, not to dismiss.”
She tells her own patients:
“I am not the final authority. If you want a second opinion, I encourage it.”
“If we can’t find an answer, we bring in more people.”
“That is the kind of culture we need in breast health:
less ego, more collaboration.”

What is breast health and why is it important?
Breast health means understanding what is normal for your body, staying up to date with recommended screening, and recognizing changes early. It’s not just about mammograms—it’s about awareness, education, and knowing when to ask questions.
What role does my OB/GYN play in breast health?
For many women, their OB/GYN is the first and sometimes only doctor they see regularly. They play an important role in early conversations about breast health, risk factors, and when to begin screening. However, additional imaging and specialist care may be needed depending on your personal risk.
What are dense breasts and why do they matter?
Dense breast tissue is very common and simply means there is more glandular tissue than fatty tissue. It matters because dense tissue can make it harder to detect cancer on a mammogram and may increase the risk of breast cancer. Can a mammogram miss breast cancer?

What breast changes should I never ignore?
You should always speak to your doctor if you notice a new lump or thickening, changes in breast shape or size, skin dimpling or redness, nipple discharge or inversion, or persistent pain in one area. Even if your imaging is normal, trust your body and advocate for yourself.
Can young women get breast cancer?
Yes. While breast cancer is more common as we age, it can occur in younger women. This is why awareness, knowing your body, and understanding your personal risk factors are so important.
How can I be proactive about my breast health?
Being proactive means knowing your breast density, staying consistent with screening, understanding your family history, asking questions, and advocating for yourself. If something doesn’t feel right, keep going until you get answers.
